The Targeted Community Care (Mental Health) Program (TCC) aims to assist people with mental illness and their families and carers.
The TCC objective is to implement community mental health initiatives to assist people with mental illness and their families and carers to manage the impact of mental illness.
The initiatives provide increased opportunities for recovery by helping people with mental illness to overcome social isolation and increasing their connections to the community with the support of personal helpers and mentors, respite services or other innovative responses.

TCC Evaluation
FaHCSIA is conducting a phased evaluation of its community mental health initiatives under the TCC to address the changing context of community mental health and to determine the role of the initiatives in the broader context of mental health service delivery.
The Phase 1 evaluation examined the three TCC initiatives: Personal Helpers and Mentors (PHaMs), Mental Health Respite Initiative (MHR), and Mental Health Community Based Services (MHCB).
