How do I locate a Personal Helpers and Mentors service provider?
The Department of Families, Housing, Community Services and Indigenous Affairs (FaHCSIA) has completed all of the four selection process for Personal Helpers and Mentors.
Service providers currently delivering Personal Helpers and Mentors are shown in the table below. The table also identifies the service provider's postcode boundaries (area serviced) and contact details.
How do I access Personal Helpers and Mentors?
People who live in a nominated postcode (as listed in the site links), can be referred to a Personal Helpers and Mentors service provider through a range of other community services, such as disability employment service providers, clinical case managers, drug and alcohol support services, or housing support services. People can also self-refer or be encouraged to access the service by their families or carers, or through a clinical pathway, such as after a period of hospitalisation. Potential participants will then undertake a functionally based assessment with the service provider to determine eligibility.

The selection of the sites by FaHCSIA has been informed through joint consultation between FaHCSIA and the State and Territory Council of Australian Governments (COAG) Mental Health Groups.
The State and Territory COAG Mental Health Groups have endorsed sites in each state and territory, based on a range of factors, including:
- demand for services
- availability of existing mental health services
- availability of clinical services for referrals (GPs, psychiatrists, psychologists etcetera)
- availability of other community services for referrals such as housing support and drug and alcohol
- availability of suitably skilled and qualified workers.
The selection of sites has ensured there is limited duplication with programs funded by other jurisdictions, and a mix of metropolitan, rural, regional and remote locations.
