The Family Support Program - Transition Plan

What is a Transition Plan?

A Transition Plan (the Plan) may be used by the Department of Families, Housing, Community Services and Indigenous Affairs (FaHCSIA) to help a Family Support Program (FSP) service transition successfully to the FSP or meet deliverables under the funding agreement.

The Plan will help FaHCSIA ensure the organisation is given appropriate support and assistance with transitioning to the FSP. It will also enable service providers to advise how they will address the issues identified by FaHCSIA during funding negotiations.

What is the aim of the Transition Plan?

The aim of the Plan is to ensure that organisations transition successfully to the FSP within 12 months. 

What should  the Transition Plan contain?

The Plan must:

  • address any of the FSP key principles and/or outcomes that FaHCSIA has advised need to be included in the Plan
  • indicate how long each strategy will take and when it will be fully implemented
  • indicate how the strategy will be sustained over the period of the funding agreement
  • be achievable and measurable, and contain clear concise statements that reflect the organisation’s capacity and priorities.

Will all FSP services be required  to complete a Transition Plan?

Most organisations will not be required to complete the Plan. FaHCSIA will discuss with individual organisations the need for the Plan during funding agreement negotiations.

When will the Transition Plan have to be developed?

The Plan should be completed and returned to FaHCSIA as part of the funding agreement discussions and before the funding offer is finalised – effective 1 July 2011.

Will the Transition Plan be reviewed?

Progress against the agreed deliverables put in place from 1 July 2011 will be reviewed before the end of March 2012.

What happens if agreed deliverables are not met?

FaHCSIA will provide guidance and work with service providers to ensure issues or difficulties meeting deliverables are achieved within the first 12 months of the funding agreement.
